Appointments for all specialists can be arranged by phoning 07 3010 5760, Monday to Friday (excluding public holidays).

As Fourth Floor Surgery is a specialist practice, patients need to have a current referral from either their general practitioner or referring practitioner. This will ensure that the Medicare rebate is applied correctly. Please arrive 5–10 minutes prior to your consultation to complete a health and personal details questionnaire.
When you come for your appointment, please remember to bring the following:
- Your referral letter from either your general practitioner or referring practitioner
- x–rays, scans and any other information you have relating to your presenting complaint
- medication history
- health insurance details or WorkCover claim number details (if applicable)
- any family members or nominated people you may wish to have present for your consultation
- a written list of questions you would like to have answered.
For all new patient appointments, every attempt will be made to contact you to confirm the appointment on the day prior. Should an appointment need to be cancelled, please do so at least one day ahead of the appointment so that other patients can be allocated.
Fees & Billing
Consultations
Our reception staff are able to provide costs for initial and review consultations at the time of booking your appointment. All consultation fees are to be paid on the day of your appointment. Fees can be paid by cash, cheque or credit card (excluding American Express or Diners).
For you to be able to claim a rebate from Medicare, you must obtain a letter of referral from an accredited medical practitioner. This is normally from either your local GP, or another specialist. Please be aware that whilst a physiotherapist can make a recommendation, this is not recognized by Medicare.
*Any medicare rebate is subject to a valid referral. Please note that a valid referral is required for the duration of your treatment. A referral from your GP is valid for 12 months. A specialist’s referral is only valid for 3 months. Please remember to check the validity of your referral for each appointment.
WorkCover
If you have a current WorkCover Claim, provided a valid claim number is supplied at the time of the appointment, we are happy to invoice WorkCover directly for your consultation. Without this, you will be required to pay a standard consultation fee and seek reimbursement.
DVA
Please provide your DVA File Number at the time of making your appointment. If necessary, we can organise transport for your appointment via the DVA Transport Scheme.
Fees for Surgery
Fourth Floor Surgery is a private specialist practice. The decision to be treated as a private patient means there will be out-of-pocket fees associated with all aspects of your treatment - including your consultations, any diagnostic tests/investigations, and any surgery that is recommended to you.
Once you have attended for a consultation and the suitable treatment options have been discussed, an estimate of your out-of-pocket fees will be provided to you. This will outline your surgeon fees, any applicable rebates from Medicare and your health fund, and the out of pocket expense you will incur.
Anaesthetist details will also be provided to you for you to obtain an estimate in relation to their fees. This will enable you to be fully prepared for the expenses that will be incurred for your proposed surgery.
